1.  Complete a Collie Club of Canada APPLICATION FOR A REGISTER OF MERIT form. (see below)

 Include a photograph and 3-generation pedigree of the candidate for the CCC records.
3.  For each titled offspring supply, on a separate sheet of paper,

    (1)  the offspring's:

        (a)  registered name,

        (b)  registration number,

        (c)  country of registration,

        (d)  sex,

        (e)  variety,

        (f)   colour,

        (g)  titles earned,

        (h)  date each title was earned

    (2)  the name & CKC/AKC/Foreign Reg.No. of the offspring’s non-applicant sire or dam, whichever applies.

4.  For all titles other than CKC Championship titles, also include either

   (1)  a copy of the certificate,

    (2)  Dogs In Canada reference where the title is published (issue, date and page) or 

    (3)  a copy of the certificate or test sheet that includes the tester’s signature.

5.       Include your non-refundable fee, payable to the COLLIE CLUB OF CANADA for the amount of $10.00 (CCC Members) or $25.00 (non‑members) for each Application. ROM and ROMX applications are considered separate applications unless submitted at the same time.

6.       Send the completed application, all required documentation, fees, photo and pedigree to:

For each titled offspring the following must be submitted on a separate sheet of paper:

Name; CKC/AKC/Foreign Reg.No.; Sex; Variety; Colour; the Name & CKC/AKC/Foreign Reg.No. of the offspring’s non-applying  sire/dam; list of titles and dates each title was earned.

For each non-CKC Championship title include one of the following: (1) a copy of the certificate, (2) a Dogs In Canada reference where the title is published (issue, date and page), or (3) a copy of the test sheet containing the tester’s signature.

Fees for each application are $10.00 for CCC Members and $25.00 for non‑members. Fees are non-refundable and must accompany this application. ROM and ROMX titles are considered separate applications unless submitted at the same time.

Make cheques and money orders payable to the COLLIE CLUB OF CANADA.  Incomplete applications will be returned & a re‑application fee of  $5.00 is required for each re‑submission.

Please also include a 3-generation pedigree and photo of the candidate for the ROM(X) album.
